Predesignation of Personal Physician
When the California Workers' Compensation Laws were amended, Organized Labor worked hard to maintain the Worker's right to choose his or her own doctor should an injury be suffered on the job. To name your current doctor by predesignation, the Predesignation of Personal Physician Form (downloadable PDF file) must be filled out and returned to your employer. Don't forget to keep a copy for your records.
